How Do I Enroll in Bridges to Access?

• There are 2 ways for patients to enroll in Bridges to Access:
  • On your own by fax or mail.
  • If you need medicine the same day, you should be enrolled via phone by an Advocate (i.e. someone such as a health care worker, social worker, or case worker, who is involved in the delivery of your healthcare).
• Either way, a completed application and income documentation is all that is needed to determine if you may be eligible for the program.
  • If enrollment documents are submitted by fax, be sure to put the applicant’s name and date of birth on each page. Also please note that faxed prescriptions are only valid if faxed directly from a physician’s office. Faxed prescriptions received from any other location will not be accepted and will delay medication shipment to the patient.
