As of January 2012, Health and Allergy Information will be required for patient enrollments and prescription processing. This information must be provided on the application. If the patient does not have any health conditions or allergies to report, the “None” box on the application must be checked. It is important that the Health and Allergy section is complete to prevent a delay in medication shipment.
