Bridges to Access is pleased to accept documents by fax beginning September 2012

As of September 2012 you may fax applications and supporting documentation to 1-855-474-3063. Please print the applicant’s name and date of birth on all documents including prescriptions*. After faxing, please allow the normal two day processing time. Once approved, medicine will be mailed to the address provided on the application.

*Faxed prescriptions are only valid if they are faxed directly from a physician’s office. Faxed prescriptions received from any other location will not be accepted.

Bridges to Access Phone Application
Advocates who call to enroll patients for same-day access to medicine will notice that the phone application no longer includes the voucher section with claim processing information. That information is available here and will also be provided to the Advocate upon completion of successful phone enrollment. The patient will use the information at a local retail pharmacy to receive up to two fills of GSK medicine.
