Patients who do not need their medicine right away can apply on their own by mailing a completed
application,
along with
proof of income
and a valid prescription to Bridges to Access. This application is available to both
patients and advocates once enrolled.

Each phone enrollment application
has a unique 9-digit identification number that serves as the patient's
Bridges to Access identification number for as long as the patient remains in the program.
Because the application has a unique ID number, it may not be photocopied.
Patients who enroll by mail will be assigned a Patient ID number when their
application for mail order shipment is processed. This type of application may be photocopied.
