Qualify and Enroll
|
- Visit Advocate with
completed form and current income documentation.
- After eligibility is granted, make sure your advocate gives you a voucher to take to any
retail pharmacy of choice.
- Bring prescription, voucher, and $10 co-pay to pharmacy
for each initial 60-day prescription per fill.

- Complete Enrollment, Patient Health and Allergy Information, and Patient Authorization to
Release and Disclose Medical Information forms with patient.
- Ensure all forms are signed by the patient, the prescriber and the advocate where appropriate.
- Call 1-866-PATIENT (1-866-728-4368) to enroll patient. You will be informed what documentation is required.
- After eligibility is granted, give the voucher to the patient to
take to any retail pharmacy of choice.
- Within 20 days, mail patient's required
documentation to Bridges to Access.

Order Refills
|
- Certain drugs are only available at your retail pharmacy. Ask
your Advocate how to get
your refill(s).
- If your refill(s) will be sent by mail order, your first refill
will be shipped to you automatically.
- 3 weeks before running out of medication, order your next refill by calling 1-866-PATIENT.
- Before requesting a refill please verify you are still eligible
for the program, there is an active drug authorization for the
medication(s) requested on file and a valid mail order prescription(s)
on file with Walgreens. Your Advocate can help with this verification.

- Advocates can order refills as well by ordering by calling 1-866-PATIENT.
- Certain drugs are only available through retail pharmacies, call 1-866-PATIENT if you need more information.
- Before requesting a refill please verify that the patient is still eligible for the program, they have an active drug authorization for the medication(s) requested and a valid mail order prescription(s) on file with Walgreens.

Re-Enroll
|
- Re-enroll in
program after one year, if necessary.
|
|
- Help patient(s) re-enroll
if assistance is still needed.
- A letter and patient-specific re-enrollment form will be sent
to the Advocate before it is time for the patient to re-enroll.
