The checklist below
will help patients prepare to apply to Bridges to Access:
Before Advocate Visit
- Find an Advocate to help you enroll in
the program.
- Download or request an enrollment
form.
- Talk to your Advocate about completing the enrollment form.
- Bring your income
documentation with you when you meet with your Advocate.
- Give your Advocate a copy of your health insurance and prescription
drug cards (if applicable).
- If you’ve applied to state or federal drug assistance programs,
bring in a copy of the application or denial letter.
- List all medications and allergies on the Patient Health and
Allergy Information sheet.
Enrollment Phone Call

Give your Advocate the required information and ask him/her to call 1-866-PATIENT before mailing the enrollment documentation.
After Enrollment
- Take prescription and Patient Voucher to local retail pharmacy
for your first 60-day prescription. (There is a $10 co-pay for each
prescription per fill at your pharmacy.)
- Remember to order refill(s)
at least three weeks before you need it by calling 1-866-PATIENT. (If a refill is needed, be sure your
Advocate sends in a mail order prescription with your enrollment
information.)

If your prescription(s) change ask the Advocate to call Bridges to Access to update your records before filling the new prescription.
- Notify your Advocate if your health insurance or income status
changes and you no longer need help from Bridges to Access.
- Call your Advocate with any questions about the program.
