For Patients Who Want to Self-Enroll

If you would like to enroll yourself in Bridges to Access, you can do so. Here's how it works:

1. Complete the Application

  • Complete, print, and sign the Bridges to Access enrollment application.
  • For help completing the application, call Bridges to Access at 1-866-PATIENT (1-866-728-4368).

Need Medicine Right Away?

If you need medicine right
away, you can be enrolled in
BRIDGES TO ACCESS®
through an Advocate by phone.

2. Gather Income Documentation

3. Mail Your Paperwork to Bridges to Access

  • Send your completed and signed application, income documentation, and prescription(s) for your GlaxoSmithKline medicines to:
    BRIDGES TO ACCESS®
     P.O. Box 29038
     Phoenix , AZ 85038-9038
  • Once your application is received and processed, you will receive a letter that lets you know whether or not you have been enrolled in Bridges to Access.
