use only. An advocate is someone involved in the delivery of the patient's healthcare services.
Unless you are a healthcare worker acting on behalf of patient(s), this section will not be of assistance to you.

Register as an Advocate and Get Program Applications

An advocate must register with our program to enroll patients into Bridges to Access. The Advocate Sign In box
on the left provides links for registering and signing in. Registered advocates can sign in with their user name and password
to access the enrollment forms which can be completed online or printed for use at a later time.

There are two types of Bridges to Access applications,
phone-in for patients needing immediate access to GlaxoSmithKline medicine(s) and
mail-in for patients who can wait to receive their medicine by mail. You can get both kinds of applications by:

Bridges to Access will send a patient-specific re-enrollment form when it is time for a patient's annual re-enrollment,
before the current enrollment expires. You or the patient should complete the form and mail it to Bridges to Access.
If the patient-specific re-enrollment form is lost, another form can be used but the Patient Identification Number
on the new form must be crossed out and the patient's actual Patient Identification Number must be clearly written.
