We have simplified the program application and now offer two kinds of enrollment for
your convenience:

Mail enrollment (new) - Patients who do not need same day access to medicine can
now enroll themselves by mail. Simply send a completed application with proof of income
documentation and prescription(s) for GSK medicine.

Phone enrollment - Prescribers/advocates with patients who need
same day access to GSK medicines can continue to use the phone enrollment process to
enroll an eligible patient.
